New Jersey, USA

About Us:

Our company's Rapid Advanced Caretm (RAC) model is tailored to the needs of frail and complex patients with higher acuity symptoms and chief complaints. By treating patients in place (generally in their home) we significantly reduce the need for the conventional acute care journey from ER to hospital admission to post acute care. Transforming the care experience - by bringing it home - radically improves patient and caregiver experiences, creates dramatic cost savings and avoids hospital acquired conditions.
For the same population of patients we also offer a proactive Recovery at Hometm (RAH) program that decreases readmissions by 30% post acute discharge. When coupled with RAC, it offers a unique readmission prevention solution for the highest risk patient population.
